If you’re caring for a loved one with dementia in Santa Barbara County or the
surrounding region, there’s a new Medicare benefit you need to know about — and a
local partnership making it easier to access.
Age Well Care has signed an agreement with Nolia Health to provide in-home respite
care under the GUIDE Model — a federal Medicare program specifically designed to
support families living with dementia.
For eligible households, that means up to $2,500 per year in covered care, renewable annually through 2032.
What Is the GUIDE Model?
GUIDE stands for Guiding an Improved Dementia Experience. Launched in July 2024
by the Centers for Medicare and Medicaid Services, it’s an eight-year nationwide
program built around a straightforward premise: people with dementia do better at
home when the whole family gets support — not just the patient.
More than 6 million Americans currently live with dementia, and the number is
expected to more than double by 2060. For years, Medicare offered very little to help
families manage that reality. The GUIDE Model is a direct response to that gap.
What Families Actually Get
Enrolled families receive a package of services that would have been out of reach for
most households before this program existed.
Each family is paired with a Care Navigator — a dedicated professional who helps
coordinate medical appointments, connect families to community resources like meals
and transportation, and simply serve as a knowledgeable guide through an often
overwhelming system.
There’s also 24/7 phone access to a care team member. For caregivers managing
middle-of-the-night crises alone, that lifeline has proven to be one of the program’s
most valued features.
And then there’s the respite care benefit — up to $2,500 every year to bring a trained
caregiver into the home, giving family caregivers a real, recurring break. Because the
GUIDE program runs through 2032, families who enroll now can access that benefit
year after year. Those who delay lose benefit years they can never recover.
Who Qualifies?
To be eligible, the person with dementia must have Original Medicare (Parts A and
B) as their primary insurance.
Medicare Advantage plans do not qualify. They must
have a physician-confirmed dementia diagnosis, must not be living in a nursing facility,
and must not be enrolled in hospice or PACE.
Note that while a family caregiver is not required to enroll in GUIDE, having one does
affect eligibility for the respite benefit specifically — and the program is designed to
support caregivers as a central part of its mission.
The GUIDE Model works alongside existing doctors — there’s no need to change any
current providers.
