Who is Get It Done SB?
Get it Done SB! is an initiative of Hospice of Santa Barbara which emerged from years of collaborative work around Advance Care Planning (ACP), the process of planning for future healthcare if you cannot advocate for yourself. We provide free workshops and one-on-one services throughout South Santa Barbara County, in English and Spanish, to help people complete and finalize their MyCare or other Advance Directive Forms.
An Advance Directive is a document for adults that indicates the types of medical care that you would like to receive if you cannot speak for yourself. It is a legally binding document that remains in effect until your wishes change and you complete a new document.
Our team is here to help our community participate in conversations regarding their future medical care with their loved ones and begin the process of completing an Advance Directive.
What are one-on-one services?
Our one-on-one services are for community members who may have busy schedules and want personalized guidance on completing an Advance Directive in a private setting.
Once you contact us, we match you with a trained ACP facilitator who will coordinate a date, time, and meeting place that works for you. Our one on one appointments commonly take place in a private room at Hospice of Santa Barbara’s offices or in a community setting (cafe, library, etc.). Staff may be available to visit your home if you are unable to travel.
In the appointment, our facilitators guide you through the MyCare Advance Directive, the most common Advance Care Planning document in Santa Barbara, and answer any questions you may have about medical treatments, quality of life, and more.
Feel free to go at your own pace; whether you schedule one appointment or three, our facilitators are flexible, afford you as much time as you need to fill out your document, and help you give the gift of peace of mind to your loved ones.
Why should you make a one-on-one appointment?
Fast and free legalization of your Advance Directive
Legalize your document in a fast and easy way right after you complete your document with us. We provide two witnesses or a free notary (upon request) at your appointment to ensure you “get it done” before you leave the appointment.
We submit your document on your behalf
No need to worry about how your doctors will find your Advance Directive. After your document is completed and legalized, we submit it on your behalf to Cottage Hospital, Sansum Clinic, and other local medical providers of your choice securely and quickly. You leave with the original document and the assurance that your Advance Directive is a part of your medical record.
