FREE Advance Care Planning Workshop
Join us for a special 2-part workshop and complete your MyCare advance directive. Advanced care planning is a way for adults of all ages, in all stages of life to prepare for the future. Advance care planning involves:
Making decisions now about the types of health care you would and would not want to receive if you become very sick or injured and cannot speak for yourself
Choosing a trusted person to make health care decisions on your behalf if you were unable to do so yourself
Talking with your doctors and loved ones about your wishes and healthcare goals
Documenting these decisions in a legal document called an advance directive. In Santa Barbara County, we recommend using the MyCare Advance Directive.
Day 1 – Wednesday, May 7, 5:30 – 6:30 PM – Overview of advance care planning, health care decisions, and MyCare
Day 2 – Wednesday, May 14, 5:30 – 7:00 PM – One-on-one help from experts and free notarization (for advance directives only). If you are updating a current advance directive or only need notarization, you have the option of attending Day 2 only.
